PBO Registration for the "2010" season

Registration cost are  league age 13-15 year olds $175 & league age 16-19 year olds $275
League age is players age on or before April 30, 2010
 

Process for mailing late registrations
1. Contact Todd or Joe at phone numbers above. ASAP!
2. Download registration forms below.
3. Complete forms.
4. Enclose a check for the registration fee made payable to "PBO"
5. Mail To:

Printing Instructions
  1. Click on each of the three forms below.
  2. Print Form 1 & Form 2 on same sheet of paper. (front & back if possible)
  3. Print Form 3 on a separate sheet.
  4. Fill out completely in ink.
  5. You must complete all three forms for each player.
  6. Bring all forms with you to registration.
